This in-person clinic is offered by Toronto Employment and Social Services in partnership with LAMP and CommuniCare. This clinic focuses on guiding you through the initial intake screening process to ensure all documentation is completed accurately and thoroughly.
Objective:
- Support you in completing the initial screening process for primary care (family doctor) attachment.
- Assist with accurate and thorough completion of detailed intake forms.
- Streamline the intake and referral process to primary care providers (family doctors).
- Enhance your understanding of eligibility requirements and next steps.
- Facilitate timely connection to appropriate primary care services (family doctor).
In-Person Clinic Location: The Queensway Employment and Social Services Office, 779 The Queensway.
Please register on this page to book a 30-minute appointment.
Walk-in service will also be available on Tuesday, May 5, from 10:00 a.m. to 4:00 p.m. on site, though an appointment is Highly Encouraged.
