This in-person clinic is offered by Toronto Employment and Social Services in partnership with LAMP and CommuniCare. This clinic focuses on guiding you through the initial intake screening process to ensure all documentation is completed accurately and thoroughly.

- Streamline the intake and referral process to primary care providers (family doctors).
- Enhance your understanding of eligibility requirements and next steps.
- Facilitate timely connection to appropriate primary care services (family doctor).
In-Person Clinic Location: York Humber Employment and Social Services Office, 605 Rogers Road.
Please register on this page to book a 30-minute appointment.
Walk-in service will also be available on Monday, May 4, from 10:00 a.m. to 4:00 p.m. on site, though an appointment is Highly Encouraged.
