In-Person Clinic to support you to secure a permanent family doctor!

This in-person clinic is offered by Toronto Employment and Social Services in partnership with LAMP and CommuniCare. 

This clinic focuses on guiding you through the initial intake screening process to ensure all documentation is completed accurately and thoroughly.

Streets to Homes Street Outreach Counsellors (SOCs), will attend this office once a month to provide Case Management support to TESS clients that have No Fixed Address and are sleeping outside.     

A settlement worker will provide a range of services for newcomers related to immigration, housing and education.
